Application for Relicensure for Board of Physical Therapy

Renewal Instructions

  1. Applicants seeking relicensure must complete the online form and pay the renewal fee online.
  2. Because state law has changed from a one-year license to a two-year license per SDCL 36-10-33, your license will be renewed for a two-year period instead of one. Therefore, you will be required to pay the “annual” renewal fee of $60 for two years for a total of $120.
  3. Continuing education required – Per ARSD 20:66:03:03, you will be required to sign a statement on the renewal application to confirm completion of 15 continuing education hours in the previous licensure period in professional education activities updating competency in Physical Therapy. Make sure you maintain a personal file of all continuing education as you may have to present proof of completion if requested by the Board.
  4. If not renewing, please notify the Board as soon as possible by selecting I will not be renewing option in the renewal form.
  5. Please review the online renewal and correct any inaccurate information, so that our database can be updated with your most current information.
  6. Prior to beginning the renewal application please ensure you have any documentation for any "yes" answers to the legal questions. The documents will be required to be uploaded.
  7. Moving forward, once renewed, your license will have an expiration date of January 1, 2026 rather than 2025. You will be required to obtain 30 hours of continuing education between January 1, 2024 and January 1, 2026. It is important to note that there are no additional continuing education requirements or fee increases at this time. The transition is from an annual renewal process to a biennial renewal process.

Updated License

If your renewal is accepted, you will be prompted to print an updated license on the confirmation page. If your renewal requires further review, you will be able to print an updated license on the website once your renewal is accepted.

During online renewal, please do not use the back button in your browser. To make a change to answers on a previous page or to review your renewal before submitting, please use the "Save for Later" button to save your renewal, log back in and access and review the previous pages. If at any time you wish to exit the process, please click the "Exit" button located at the bottom of the page.

If you will be away from your computer for any period of time after starting your online renewal, it is strongly advised that you click on the "Save for Later" button located at the bottom of each page to minimize the risk of losing the data that you have input into the system. You can then log back into the renewal at a later time by logging into the online renewal.
