Renew your license online.

Note: Renewals are available beginning September 15th.

If you are not renewing online please complete the paper form located here. You must include a check or money order with the paper form.

Print Licensure Card

Print an updated certificate for your records.

Note: The board will no longer mail updated seals and it will be the licensee's responsibility to print a new license with the updated expiration date.
